Why does MCL relapse after many years?

Even if additional treatment is needed, there are a number of options available for people with relapsed or refractory mantle cell lymphoma (MCL). But to answer this question: Although MCL often responds well to the first treatment, over time MCL can relapse, or recur, because small areas of cancer cells may remain in the body after treatment. Over time these cells may multiply and grow, causing the cancer to return. The length of remission (disappearance of signs and symptoms of cancer) after treatment may vary because of things like the biology of the lymphoma or the kind of treatment initially given.
